This is a personalised voucher. Enter a voucher amount for dental treatments. The beneficiary will receive the chosen dental treatments at the practice. The difference will be paid in the surgery, as appropriate, or kept in the beneficiary’s account. All other conditions also apply to this type of voucher.
1. The voucher can be used within 6 months of purchase for any dental treatment.
2. If the requested dental treatments exceed the amount on the voucher, the difference is paid in the surgery.
3. If the requested dental treatments cost less than the amount on the voucher, the money is not refunded but will be kept in the beneficiary’s account for 6 months after the purchase of the voucher.
4. The voucher is non-transferable and can only be used by the holder.
5. The voucher is not redeemable for cash.
